IMPORTANT: Effective beginning on January 1, 2011, over-the-counter medicines or drugs (e.g. Advil, Ibuprofen, cough syrup) are not eligible for reimbursement under an FSA, without a doctor’s prescription. Insulin is the only medicine that doesn’t require a prescription. Also supplies that you need for medical care (e.g. contact lens solutions, bandages for wounds, thermometers) will continue to be eligible for reimbursement.
As such, upcoming enrollment in your employer’s flexible spending account should take into consideration as of 1/1/2011, over the counter items will no longer be reimbursable under the health FSA.

Important Things to Know for 2008-2009

  1. Bridgeport ISD Cafeteria plan year is September 1, 2008 to August 31st , 2009.


  2. MGM is the Third Party Administrator (TPA) for Bridgeport ISD for plan year September 1, 2008 through August 31, 2009.


  3. Flexible Spending Account (FSA) A new MGM Visa Card will be issued to all participants of the medical reimbursement accounts for a $1.50/month fee.


  4. Dental: OraQuest Dental is the dental provider for Bridgeport ISD effective 09.1.2008. To view pan summary and rates, click on Benefits and Forms/Dental..


  5. Vision: Block Vision is the Vision provider for Bridgeport ISD effective 09.1.2008. To view plan summary and rates, click on Benefits and Forms/Vision.


  6. Disability: Unum Provident will continue to offer disability for Bridgeport ISD effective 09.1.2008. Employees can enroll on a guarantee issue basis (no health questions asked).   7. Voluntary Life: Fort Dearborn Life is the new voluntary life provider for Bridgeport ISD effective 09.1.2008. Employees can enroll for 7 times salary up to $500,000 for self, 100% of employee election for Spouse and $10,000 for dependent children. Employees can also purchase AD&D coverage. Employee benefits are available up to a maximum of $500,000. Employee spouse benefit is equal to 50% of the employee amount and the child benefit is equal to 10% of the employee amount.


  8. Individual Permanent Life: Texas Life will continue to offer Individual Permanent Life insurance for Bridgeport ISD. This insurance offers level term coverage to age 110 without any benefit reductions. To view plan summary and rates, click on Benefits and Forms/Individual Permanent Life.
